Navigating the Spanish Healthcare System

Spain has excellent public healthcare, but accessing it as a foreigner has specific steps. Here's how to register and what to expect.

Spain's public healthcare system (Servicio Canario de la Salud in the Canaries) is ranked among the best in the world. As a registered resident, you can access it. But 'registered resident' is the key phrase — you need your paperwork sorted first.

Getting your tarjeta sanitaria

You need: your NIE, your empadronamiento (proof of address from your local ayuntamiento), and your social security number. If you're employed in Spain, your employer handles the social security registration. If you're self-employed (autónomo) or retired, you register yourself at the Tesorería General de la Seguridad Social. Once you have all three, visit your nearest Centro de Salud and request your tarjeta sanitaria — the health card that gives you access to the system.

What to expect

Your Centro de Salud is your first point of contact — a local clinic where you get a assigned GP (médico de cabecera). Appointments are usually same-day or next-day for your GP. Specialist referrals take longer — weeks to months depending on urgency. Emergency rooms (urgencias) are available 24/7 at the main hospitals. The Hospital Universitario de Gran Canaria Doctor Negrín and Hospital Universitario Insular are the two main ones in Las Palmas.
